WHAT YOU'LL DO
- Get to know new patients and their families, and figure out what they need to thrive at home.
- Build personalized care plans with patients and families, not just for them.
- Provide hands-on nursing care, including medications and treatments as prescribed.
- Teach patients and families how to feel confident managing their health.
- Partner with physicians, community resources, and home health aides to keep care coordinated.
- Guide and support our home health aides.
- Help plan for a smooth transition when a patient is ready for discharge.
WHO WE'RE LOOKING FOR
You'll need:
- A degree from an accredited school of nursing.
- A current state RN license and CPR certification.
- A valid driver's license and a reliable, insured car.
- Strong observation, communication (verbal and written), problem-solving, and basic math skills.
- The ability to do the hands-on parts of the job: standing and walking for long stretches, lifting supplies and equipment, helping patients move or transfer safely, and reaching, bending, and kneeling as needed.
- Good vision and hearing to perform nursing skills safely.
GOOD TO KNOW
This is a field-based role, so you'll be driving to patients' home within our service area. As with any nursing job, you may be exposed to infections and contagious diseases, and no two situations are the same. You'll be ready for the unexpected, including the occasional emergency.

Compensation: $125,000.00 - $145,000.00 per year
